The twins kinda act like that as a way to make fun of the incest troupe, it’s definitely weird to watch but it’s not actually meant to be taken seriously. There’s some problematic things in Ouran but if you can get past those it’s a great watch!

I'm pretty sure the twins aren't actually like that,they just act like it for the girls at the host club
